Records Request

We are required to retain your records for seven years from the date of termination of services. Texas law requires that record requests be made in writing. To obtain a copy of your records, please submit your request to hello@vivawellcounseling.com.

The administrative fee for this request is $25.

Business Violation Concerns

If you have a consumer complaint regarding the privacy and security of your health information, you may contact the Texas Office of the Attorney General and file a consumer complaint.
